AS THE PARENT/GUARDIAN,
I understand Boys & Girls Clubs of the Gila River Indian Community (BGCGRIC) is not a licensed Child Care Facility under ARS Section 36-882. Members are responsible for signing themselves in and out of the facility daily. It is the responsibility of the parent/guardian to advise the child whom they may leave with if it is someone other than a parent or guardian.
I understand that BGCGRIC has a Safe Passage Policy, and that once a child under the age of 14 has entered the building, they will not be allowed to leave until an authorized adult retrieves them from the Front Desk. Verbal or call-in permission is not permitted. Members ages 14 and older may leave the Club unescorted with the completion and submission of the Safe Passage Consent Form. Members from the same household ages 14 and older may also escort younger members from BGCGRIC with the completion and submission of the Safe Passage Consent Form. Once a member of any age checks out of the program, they must leave the property and will not be allowed to return for the day unless accompanied by an authorized adult. Safe Passage Consent Form may be obtained from the Front Desk.
- I understand that Membership dues are valid August 1 – July 31 regardless of enrollment date. Additionally, separate fees for monthly Afterschool Program, Summer Program, Sports Leagues, Field Trips, or any fee-based program are due prior to enrollment and attendance. Payment due dates differ for Afterschool Program and Summer Program.
- Afterschool Program registration fees are due on the 20th of each month for the following program session.
- Example: Fees for October are due September 20th. Registration payments not received by the 25th of each month, are subject to a $10 late registration fee per child.
- Summer Program registration fees are due weekly on Wednesday for the following program session.
- Example: Fees for the week of June 11th are due on June 6th. Payments not received by Wednesday of each week, are subject to a $10 late registration fee per child.
- If the due date falls on a weekend or holiday, fees are due the business day prior. Late registration fees must be paid prior to continued attendance in the program.
IMPORTANT: All payments are final. There are no refunds or pro-rates after payment is received. Failure to pay on-time will result in additional fees and/or suspension from the program.
- If/when information regarding a member changes it is the parent/guardian’s responsibility to inform Club personnel. Changes may include: phone numbers, address, school, emergency contact information, etc. Failure to provide updated information is a matter of safety and my result in suspension from program.
- Club hours of operation vary and may include holiday and staff training closures. It is the parent/guardian’s responsibility to know the Club schedule and arrange transportation to and from the Club. A $10 late pick up fee per child will be assessed for every block of 15 minutes a member is left past Club hours. Late fees must be paid prior to the member returning to the Club. If a member is left past closing more than 3 times within a 12 month period, membership may be suspended for a minimum of 1 week. If a child is left for more than 30 minutes past closing and Club personnel have exhausted all measures to locate parent/ guardian, local authorities may be notified.
- A member may not attend the Club if they have not attended school for any reason. This includes, but is not limited to: illness, truancy, or suspension. In addition, if a child becomes ill while at the Club, a parent/guardian will be notified and must, upon notification, make arrangements to pick up the child immediately. If a member has been exposed to or contracts a contagious disease the parent/guardian must inform Club personnel immediately. Member my not attend the Club until they are no longer contagious. Written authorization from a treating physician may be required to return.
- BGCGRIC reserves the right to suspend any member from the Club or Club activities for exhibiting poor behavior. Acts of violence or bullying may result in immediate suspension and/or expulsion. It is the parent/guardian’s responsibility to meet with Club personnel regarding concerns on behalf of either party.
- Staff reserve the right to restrict usage of cell phones and other electronic devices by members. No pictures, video or audio recording by members is allowed, and may result in confiscation of equipment, suspension of member, and in illegal instances, contacting local authorities. Additional practices may vary at individual locations. Questions regarding rules and expectations should be directed to Club personnel.
I understand the rules of the Boys & Girls Clubs of the Gila River Indian Community (BGCGRIC) and request that my child be admitted into membership. I agree that BGCGRIC will not be responsible for personal injury or lost, missing or damaged property while on the Club premises or engaged in any of its activities away from the Club. I have explained the rules to my child, and agree to the terms listed above and those included in the Parent Orientation/Policy Handbook. I agree to ensure that all information provided to BGCGRIC is current and accurate.
While I understand that every effort will be made to reach me, I give my consent to have my child be given a medical examination and/ or emergency treatment by a paramedic, physician or hospital in case of emergency.
