I, Full Name*, owner of Cornhole Club/Company here by give Camron Clandening owner of Synergy 6 Promotions permission to sell and promote my product.
I Full Name* , am aware that this is a 12 month contract starting at the following first of the month Following Month*. I am aware that Synergy 6 will be promoting and making sales on my behalf. I am also aware of the per item payouts and understand that I will not receive a payout statement until at least the 5th of the following month.
I understand that my startup fee of ($250) will give my club/company the following items for the following 365 days.

Items:

*Public store for Customers/Club Members on Synergy 6

*1 QR Code for directing Customers/Club Members to Synergy 6

*2 Discount codes for your personal club/company collection (10% and 20% OFF).

*1 Media Launch Day on Facebook & Associated sites/groups.

*Unlimited Revisions on items on Club/Company Collection.

*Unlimited Logos.

*24/7 Tech Support.

*1 BASIC digitizing of club logo for embroidery. High res, multiple colors, and detailed logo's cost extra ($200) Includes 10 hats!

Official Image/Logo Release Form
I, Full Name*, copyright holder of all Club/Brand Name* logos, hereby grant permission for OG APPAREL to digitize all logos for the purpose of embroidery.
I also give permission to Synergy 6 to use our logo for any merchandise that would financially benefit Synergy 6 and Club/Brand Name*.
By signing this waiver you are agreeing to work in partnership with Synergy 6 using their drop shipping company platform.
