IMPORTANT SPECIAL NOTICES
Please submit all documentation online at least seven (7) calendar days before the day of the Dan Promotion Exam. If this is not possible bring the documents to the Exam. Any missing documents, must be received within fifteen (15) calendar days following the exam. Results will not be recorded, and Certificates will not be mailed until all requirements have been met. A Late Fee ($10.00) will be assessed.
Full Payment must accompany the Application in order for it to be submitted to the Board for consideration and approval.
In the event that an application is not approved, the test fee will be refunded to the account used for the transaction.
Withdrawal of Applicant/Application is subject to a service charge ($50.00 minimum) except in the case of severe injury, illness, death in the family, airline failure, or act of nature, all of which must be documented. Service Charge is at the sole discretion of the USAT Martial Arts Commission.

If the applicant is not a U.S. Citizen, he/she must submit proof of U.S. Residency for at least six (6) months.
If the applicant has a prior certificate from a credible organization, a copy of that certificate must be submitted with this application.
The instructor of the applicant should insure that no false statement will be found on the application; otherewise, the Dan will be nullified even after its issuance & the instructor will not be permitted to make further recommendations. There will be NO REFUNDS.
If anything is missing from the application packet, the applicant must submit the missing item(s) within 15 days following the Exam. Otherwise it will not be processed.
No parent or student should call the USA Taekwondo or the USAT-MAC Dan Office. Only instructors are permitted to communicate with the Dan Office.
