IMPORTANT SPECIAL NOTICES

Your request cannot be completely evaluated until we have all the materials requested.

Once you are recommended for a specific Dan Rank, you will be notified at the email address you have provided. The materials you have already submitted will be accepted as your completed application. We will send you an electronic statement for the Testing Fees.

If applying for 2nd Poom/Dan or higher, please complete the following sections.

Education & Experience (schooling, competitions, seminars, coaching, refereeing, ...) Give (1) approximate date, and (2) name/brief description of event.

Taekwondo Records (medals, awards, certifications ...) Give (1) approximate date, and (2) name/brief description of event.

Upload your Personal Photo, Certificates, Instructor Recommendation and Resume (Martial Art Activities; Other Career; Community Service) These documents will be required to complete your application. If you cannot upload them now, please prepare to e-mail (PDF), FAX, upload or mail them as soon as possible.

IMPORTANT SPECIAL NOTICES

Please submit all documentation online at least seven (7) calendar days before the day of the Dan Promotion Exam. If this is not possible bring the documents to the Exam. Any missing documents, must be received within fifteen (15) calendar days following the exam. Results will not be recorded, and Certificates will not be mailed until all requirements have been met. A Late Fee ($10.00) will be assessed.

Full Payment must accompany the Application in order for it to be submitted to the Board for consideration and approval.

In the event that an application is not approved, the test fee will be refunded to the account used for the transaction.

Withdrawal of Applicant/Application is subject to a service charge ($50.00 minimum) except in the case of severe injury, illness, death in the family, airline failure, or act of nature, all of which must be documented. Service Charge is at the sole discretion of the USAT Martial Arts Commission.

Applications may be accepted at the door. A Late Fee ($10.00) will apply. As above, all documentation must be received within fifteen (15) calendar days following the exam. Results will not be recorded, and Certificates will not be mailed until all requirements have been met.

If the applicant is not a U.S. Citizen, he/she must submit proof of U.S. Residency for at least six (6) months.

If the applicant has a prior certificate from a credible organization, a copy of that certificate must be submitted with this application.

The instructor of the applicant should insure that no false statement will be found on the application; otherewise, the Dan will be nullified even after its issuance & the instructor will not be permitted to make further recommendations. There will be NO REFUNDS.

If anything is missing from the application packet, the applicant must submit the missing item(s) within 15 days following the Exam. Otherwise it will not be processed.

No parent or student should call the USA Taekwondo or the USAT-MAC Dan Office. Only instructors are permitted to communicate with the Dan Office.

You must complete Poomsae Seminar in order to apply for Dan Rank Test.

You may order Official USAT-MAC Taekwondo Uniform and Embroidered Belt. Please provide uniform and belt size below. Normal delivery time is 4-6 weeks.
