All ballet performance participants are required to provide their own tights and footwear per the Costume Directions (Nutcracker - TBA in Sept Party girls, little sisters and Clara, will be required to purchase a hair piece through CDT. (estimated cost $15-20+tax))

Ballet Performance: I give permission for my child to participate in CDT’s production as indicated above. I have reviewed the rehearsal, dress rehearsal and performance show dates will make them a top priority. I also agree to download the Team App to my apple or android product for communication purposes.

Nutcracker: I agree to pay the $20 per month participation fee for a single role or $10 per month/per role if cast in multiple parts. This fee will begin in August and end in December; this will be processed in Team app directly to the Children's Dance Theater Foundation. I agree to get the necessary items by October 1st.

Spring Ballet: I agree to pay the $20 per month participation fee for a single role or $10 per month/per role if cast in multiple parts. This fee will begin in February and end in May; this will be processed in Team app directly to the Children's Dance Theater Foundation. I agree to get the necessary items by March 1st.

All applicants: Finally, the undersigned parent/student indemnifies and agrees to holds Children’s Dance Theater (CDT), its owner, staff, instructors, agents, volunteers and assigns from any and all liability whatsoever, for any damage, sickness (including COVID-19) or injuries, and from any and all claims and demands, including attorney fees, arising out of the party’s participation in dance lessons, classes, workshops, performances, fundraisers and other related activities. The undersigned parent/student understands that students may occasionally appear in promotional performance videos, photos, brochures, and other materials as a result of his/her association with CDT. By registering a child/student for class, such use of the child/student’s name and likeness are agreed to and acknowledged and, accordingly, all right, title and interest in same are waived.

I understand that auditions are selective and choreographers and directors will cast as they see fit. Casting decisions are final and I agree and accept their decision. The pillars of being an Elite team member are teamwork, dedication, work ethic and perseverance. With the Covid 19 situation affecting our dance world so drastically can you describe how you are demonstrating those core values in your dance classes and in other facets of your life.
