Employee Acknowledgment of Receipt of Policy Against Unlawful Harassment, Discrimination and Retaliation and Personal Appearance and Behavior Policy

This will acknowledge that I received a copy of the Policy Against Unlawful Harassment, Discrimination, and Retaliation and Personal Appearance and Behavior Policy and that I will comply with its requirements. I understand that under California law, I will be held personally liable for harassing conduct that violates the California Fair Employment and Housing Act.

This will acknowledge that I received a copy of the Notice Regarding Unlawful Harassment, Discrimination and Retaliation and that I will comply with its requirements. I understand that under California law, I will be held personally liable for harassing conduct that violates the California Fair Employment and Housing Act.

COMPANY EQUIPMENT POLICY

I acknowledge that while I am working for Davis Trucking, LLC ("Davis Trucking"), I may be using Davis Trucking's equipment and property, including but not limited to, cellular phones, laptops, computers. I agree to take proper care of the property that I use. I agree that upon termination of my employment or earlier request by Davis Trucking, I will return any and all property of Davis Trucking in proper working order and in the same manner in which it was given, excepting normal wear and tear.

I acknowledge and agree that I am responsible for reporting any damage or loss to such property immediately and that I may be held financially responsible for damage or lost property.

My signature below attests to the fact that I have read, understand and agree to be legally bound to all the above terms.

Safety Footwear Policy

 

Davis Trucking LLC strives to take all reasonable precautions to protect employees from physical harm that might result from routine work activities. Accordingly, an employee's footwear must be compatible with the work assignment. Some work assignments may consequently require the use of safety footwear.

Safety footwear shall be worn in All warehouses, Davis vehicles, and any other areas as determined by supervisors. All safety footwear shall comply with American National Standards Institute (ANSI) Standard ANSI Z41 - 1991, American National Standard for Personal Protection-Protective Footwear.

Safety footwear with impact protection are required to be worn in work areas where carrying or handling materials such as packages, objects, parts, heavy tools, or any other activities where objects might fall onto the feet. Safety footwear with compression protection are required for work activities involving skid trucks or other activities in which materials or equipment could potentially roll over an employee's feet. Safety footwear with puncture protection are required where sharp objects such as nails, wire, tacks, screws, large staples, scrap metal, etc., could be stepped on by employees causing a foot injury.

Composite/Steel-reinforced Safety Shoes: These shoes are designed to protect feet from common machinery hazards such as falling or rolling objects, cuts, and punctures. The entire toe box and insole are reinforced with steel, and steel, aluminum, or plastic materials protect the instep. Safety shoes are designed to insulate against temperature extremes and may be equipped with special soles to guard against slip, chemicals, and/or electrical hazards.

D. Responsibilities

1. A list of employees and/or job operations requiring safety footwear will be kept by both the supervisor and department management.
2. Employees are expected to wear safety footwear that is appropriate for the areas where they are assigned to work.
3. Employees are expected to care for, clean, and maintain safety footwear as required.
4. Persons employed as contract labor are expected to wear appropriate safety footwear as required in this procedure.
