APA Online Award Request Form
This Request Form is only for players that have received an award and wish to claim that award. Any award will include a patch and memory plate that will be delivered in the mail. Please be sure to have your contact information updated in the APA Member Services so that you will receive it in the mail.

Once requested, awards will be available at the next APA hosted Tournament or will be delivered directly on League Night. Please note that the name on the memory plates will be exactly as it is on the scoresheet with nicknames included.

Players must be up to date with all League payments to receive the award.
