  • We are a preferred provider for Regence Blue Cross Blue Shield of Oregon, Moda, Cigna, Providence, Kaiser, PacificSource, and CareOregon. Sunbreak Therapy Services will bill these insurance companies for your sessions. For other insurances, we will bill the insurance plan for out of network services; however, you are responsible for your bill. Please be advised that it is your responsibility to check your benefits. Our office requires a referral from your doctor prior to the first visit.

    Self-Pay Discount

    If you are not submitting to your private insurance for reimbursement, we will offer a discount for self-pay. Your card will be kept on file to be run at the time of service or you may pay by check/cash. 

    Returned Check Policy

    Checks that are returned for insufficient funds will incur a $50.00 charge.

    Attendance Policy

    Every effort will be made to schedule a time convenient to your schedule. Notification of cancellation should be at least 24 hours prior to your appointment. We will make exceptions for sudden illness. A charge of $50.00 will be incurred for a no-show appointment or a late cancelled appointment. You are responsible for the charge. Insurance will not pay.

  • I have read these policies and understand that I am solely responsible for any charges incurred at Sunbreak Therapy Services. I give my written permission for Sunbreak Therapy Services to treat myself or my child and communicate with my insurance as necessary for billing and payments.
