Completing this online form will send a request to the Graduate Advisor to manually set up the requested form(s) as a DocuSign envelope on your behalf. Submitting this online form does not automatically create a DocuSign envelope.
Department forms are available for download on the Anthro Student Resources page> Forms. It is not required to complete and upload the form here, but doing so helps speed up the set up process.
Note - If a form requires student signature, do not sign it before upload. This is especially important for DGE forms as only certain types of electronic signatures are considered valid. Student signature will be obtained in DocuSign, which meets their requirements.
Once the form is sent via DocuSign, students will be able to fill out the form, sign, and monitor signatures. It is the student's responsibility to give their committee advance notice of the DocuSign email and follow-up on pending signatures. Instructions to send to the committee will be available after submitting this form.
