Community Gives Youth Service Program Application

The CommunityGives Youth Service Program is supported by Dow Company and the Midland Area Community Foundation as an opportunity for K-12 teams, clubs or groups (sports, band, robotics, etc) in Midland County to earn funding for team needs through volunteerism. Please note that this submission – once approved – is considered your contract.

That being said, you should make tentative arrangements with a local charitable organization for your volunteer project before you apply. Remember, your project should provide at least 5 youth with about 3 hours of time spent in a meaningful project, and must take place within 6 weeks following your submission of this application. Please allow at least 1 week between your application and the event in order for the program administrator to review your application and to help alter plans should it be necessary. You should receive an e-mail confirming receipt of your application from the JotForm program and within 3 business days from program administrator with any suggested changes.

Your Final Report should be completed within 6 weeks of project approval and within 2 weeks following the completion of your proposed project.

Eligible Groups must be:

Made up of youth in the K-12 Age Range
Located in Midland County and comprised of predominantly Midland County residents
Affiliated with a 501c3 or a school district
