• Community Gives Youth Service Program Final Report

  • Your Final Report (including at least 3 high-quality photos and MACF Photo Release Forms for EACH volunteer) is due within two weeks of your approved and completed volunteer project and should reflect the information provided in your application.

    Your approved application is considered a contract, and your application was evaluated on the information you provided there - the demonstrated group need, the impact of the proposed volunteer project, the number of K-12 volunteers participating, and so forth.

    These grants are competitive, and 10 fewer volunteers (for example) would have changed your group's standing in the evaluation process.  Changes to the proposed and approved application may invalidate said contract.

    Unless otherwise arranged with Sara JacobsCarter, CommunityGives Youth Service Program Coordinator, checks will be sent to the address of the Fiduciary Organization.
