Would-be field trip participants without signed waivers will not be permitted to take part in the field trip. “Minors” (under 18 years of age) must have Parents or Guardians sign this Agreement for them. Participants must consider the general and specific risks listed below, and assume the risks inherent with the activity, as a condition of participation in the field trip.

I, the undersigned, am aware of and understand the risks, hazards and dangers inherent in field trips such as, but not limited to, the possibility of personal injury, death, property damage or loss, resulting from participation in field trips, and/or travel to and from the location of a field trip. Notwithstanding the risks involved, I request the YNC to permit me and/or the Minors accompanying me to participate in this field trip, and I hereby assume all risks resulting from engaging in such activity, including travel to and from the location of the field trip.
I hereby acknowledge that I have read this Agreement and understand that it is a full and final release and waiver of any claims for damages or injuries sustained as a result of our participation in this field trip, and/or travel to and from the location of the field trip, and that I am signing this Agreement voluntarily, knowing that, in doing so, I am relieving and forever discharging the Young Naturalists Club, their agents, servants, directors, officers, successors, and assigns of all liability for any losses or damages which I or the Minor may sustain.
