I understand that I am applying as a 1099 contractor responsible for my own taxes, insurance, and other delivery driver requirements. I understand I am not employed by ACK Eats LLC.
I certify that I have and will maintain an auto insurance policy under my name for the full extent of the time I am a delivery driver partnered with ACK Eats LLC.
I understand that I will be leasing delivery equipment from ACK Eats for which I shall pay $2 per week. I understand that it is my responsibility to return the equipment via certified mail, or in person in the event I stop driving.
