This form is used for matches involving an ESFA club on ESFA grounds. If your opponent is from another Assocation you will not be able to submit the form without uplocading a Football NSW Inter-Association Trial Match Form.

• This form must be submitted to ESFA seven (7) days prior to any trial match if referees are required.
• If referees are not required then this form must be submitted a minimum of two (2) days prior to the first match on this form.
• Clubs competing in matches outside of ESFA against teams from other Associations must submit this form seven (7) days prior to the match together with the Football NSW Sanction Form.
• Please note that referees cannot be guaranteed.  Clubs must not approach referees directly to cover matches.  Referee fees are to be paid in cash on the day prior to commencement of each match.
• Please ensure that you have confirmed ground bookings.

Please ensure that you use correct field names.  For example Mission Fields are either Centennial Park 09 or 10.
