PLEASE CAREFULLY READ AND SIGN THE FOLLOWING RELEASE:
I agree to hold harmless CCS, their officers, directors, employees, and volunteers from all claims, liability, or lawsuits arising from any accident or injury from my child’s actions while participating in a CCS Summer Program. In case of emergency, Charleston Collegiate School’s procedure is to contact the parent at home or work. If unable to reach either parent, the school will contact the next Emergency contact listed. In case of serious accident or illness, the school may contact EMS if the situation warrants emergency treatment. I will not hold the school financially responsible for any emergency care or transportation of my child, in the event that emergency treatment is needed.
COVID-19 DISCLAIMER– CCS takes the health and well-being of its campers and counselors very seriously. With the ongoing COVID-19 activity, CCS will be taking appropriate measures to try to prevent and mitigate possible exposure during summer camp operations. All families registering for summer camp understand that they are choosing to participate at their own risk. CCS will not be held liable if families choose to participate in summer camp.
Additionally, families must understand that everyone has a responsibility to ensure campers are healthy enough to participate in summer camp. Anyone with a temperature above 100 degrees, or exhibiting a persistent cough, will not be allowed to participate in camp until such symptoms subside. An emergency contact must be in place to pick up any camper deemed sick.
CCS may use your child’s photograph in-school promotional materials. If you do not want your child’s photo or work used in this way, please send a written notification to the school.
