    This is a combined application for both the Northwest CO Incident Management Team and the Pitkin County Incident Management Team. You will be asked to indicate which team you are applying for below. 

    All individuals applying for positions on the Northwest Colorado Type 3 Incident Management Team or trainee positions should submit this nomination form and a copy of their current fire qualifications (Red Card) or Colorado Type 3 certificate as soon as possible. A "File Upload" section is provided to do that. 

    Current NWIMT & Pitkin IMT members are required to submit this form to ensure correct personal information and to secure permission from your home agency. For any positions not listed on your Red Card, please attach a copy of the completed Task Book final evaluator, agency signature page, and your highest ICS Course such as I-300. The applicant's supervisor / Chief must approve all nominations. Some Units may require a second level of approval (i.e. Fire Chief; Forest / Park FMO or State).

  • Please list the ICS position(s) for which you are qualified (have a completed taskbook). Be sure to note the exact month, day and year of effective and expiration dates. Also note if it is a NWCG or FEMA / Colorado All Hazards.

  • Please list the ICS position(s) for which you are a TRAINEE. You are considered a trainee ONLY if you have an open & current (w/in expiration period) position task book.

    You MUST indicate the exact month, day, and year you INITIATED your position task book AND the current percentage of completion below. 

    Having this accurate information allows the team coordinators to ensure you are exposed to trainee opportunities so that you gain proficiency in a position and qualification.

    Using the checkboxes below, please mark the courses you HAVE attended. If you attended a wildland (NWCG) equivalent course please mark it in the All-Hazards course area. The course number may be an L, E, or P listing; AH stands for All-Hazard.

  • INCIDENT ASSIGNMENT INFORMATION

  • If you are currently QUALIFIED OR LISTED AS A TRAINEE in an ICS position (IC, resource unit leader, etc.) AND you are NOT listed in ROSS/IROC by your home agency you MUST upload one PDF document with all the appropriate documents in support of your listed qualified position.

    Per the 2022 IQSWeb Terms and Agreements, qualified or trainee IMT members must provide the following documentation supporting their qualification or trainee status. At a minimum, these records shall include the following:

    a. Completed Taskbooks (Pages 1, 2 and evaluation records)
    b. Initiated Taskbooks (Page 1)
    c. NWCG Training Certificates
    d. Experience Records to document proof of currency required for qualifications. For
    example, Resource Orders, CTRs, Evaluations, DFPC IQS Experience Form*, etc.
    *DFPC IQS Experience Form can be found on the DFPC website.

    Although the IQSWeb system is specific to NWCG qualifications, for members with ALL-HAZARD qualifications to be listed in IQS for deployments to wildfires (as tech specialists), the supporting documents must be uploaded.

    To assist you in determining which "Training Certificates" you may need to upload, please click on the following LINK. The documents/certificates are listed, by item, under "Prerequisite training completed" and "Prerequisite qualification(s) required to be completed." A completed course certificate may be required for each class/pre-requisite listed. Each ICS position is listed SEPARATELY in the linked document below.

  • The applicant must provide the permission of their supervisor / Chief who has the authority to allow them to be away from their daily assignment for extended times. The applicant will be expected to attend assignments and team training to develop their skills as members of this regional AHIMT.

  • SUPERVISOR/CHIEF INFORMATION

  • I concur with the goals, commitment, and availability of the applicant for disaster/emergency assignments as a member of the Northwest Colorado Type 3 All Hazards Incident Management Team. The applicant has permission to leave their work assignment to participate. I also certify that the participant's basic insurance requirements (including but not limited to workers compensation and liability) are covered by their home agency.

    ** Special note about mutual aid and reimbursable incidents:

    Most of the incidents that the Northwest Type 3 IMT responds to are considered mutual aid and will follow protocols established in the local Annual Operating Plans, or other mutual aid agreements. However, some incidents may be eligible for reimbursement depending on ownership and/or a disaster declaration. In these cases payment procedures will be established at the time of the incident.
