Guest Teaching Contract

This agreement is between client participant (“Client”) and guest teacher, MaryAnn George, (“Guest Teacher”) in regards to services provided by Teacher. This Agreement (“Agreement”) applies to all services provided by the Teacher to the Participant.

When I have MaryAnn George as a Guest Teacher, I am aware of and agree to the following terms (please check each box to agree):*

In exchange for each one hour workshop, I agree to pay $3,600 within 21 days of the completed class.I agree that materials produced and shared by Guest Teacher will only be used within the context of these sessions. I agree that materials shared by Guest Teacher will not be used to create or offer classes without the awareness and participation of Guest Teacher.I agree to discontinue use of any class recordings after 5 months of the initial class date, unless otherwise discussed and agreed upon with Guest Teacher.I understand Guest Teacher does not provide medical, therapy, or psychotherapy services. I understand Guest Teacher is not responsible for any decisions made as a result of this teaching and/or any consequences thereof.I agree to indemnify, defend and hold harmless Guest Teacher from and against any losses, claims, liabilities, expenses or damages, including attorneys’ fees and costs, incurred in connection with the defense of any third-party claim brought on account of my engaging in this teaching.I agree that all disputes arising under this agreement shall be governed in accordance with the laws of the state of Missouri and that the parties to this agreement shall submit all disputes arising under this agreement to a single arbitrator in the City of St. Louis, Missouri, for resolution by binding arbitration

THIS CONTRACT CONTAINS A BINDING ARBITRATION PROVISION WHICH MAY BE ENFORCED BY THE PARTIES.
