By entering his/her signature, VOLUNTEER and PARENT/GUARDIAN of volunteer if under 18, hereby agree to the following terms and conditions:

I understand that I will provide volunteer care for dog(s) from One Upon a Tail Rescue, as well as time and any services, on a volunteer basis and at no cost/charge to the rescue.  I also agree to the following conditions while Volunteering:

      • I understand that I represent the rescue when volunteering, and agree to dress appropriately, speak and act in a professional manner, and wear a mask as mandated by local and/or federal law.
      • I will treat all customers, visitors, potential adopters, venue staff, other volunteers and rescue personnel with respect and kindness at all times.
      • I understand that I may have access to personal information about potential adopters in the course of my volunteer work, and that information is confidential and not to be shared.
      • I understand that many of our pups have been abused, traumatized, untrained, unsocialized or had little human contact in the past. I will provide patience and positive reinforcement, as EVERY human interaction is highly important to helping them heal from their past and secure a forever home!
      • I understand that the prior history of rescue dogs can make them fearful, prone to try to escape or run away by darting, pulling, twisting, turning or any means possible, and I will take all necessary precautions to esure safety.
      • I agree to review the kennel card of each dog before taking it out, to learn about them, particularly if they are DOG FRIENDLY or NOT.
      • I agree to ALWAYS use a Slip Lead (or Harness for the bigger dogs), as collars may not always be sized right or secure, and to keep it supervised at all times.
      • I agree to always secure the kennel door with the left foot, and ensure that the leash/harness is secured on the dog, before fully opening the kennel door.
      • I agree to ask for assistance when needed with a dog, visitor or potential adopter and will not answer any questions that I have not read on the kennel card or been trained to provide.
      • I agree to volunteer with the dogs and rescue at my own risk, and indemnify and release Once Upon a Tail Rescue, it's Officers, Directors, and volunteers, from any and all liability arising from damages to person(s), pet(s) or property caused by the rescue dog(s) or during my volunteer work.

PUPPY & UNVACCINATED DOG SAFETY PRECAUTIONS

I further AGREE to the following statements about KEEPING PUPPIES, UNVACCINATED DOGS OR NEWLY VACCINATED DOGS SAFE, and have READ THESE STATEMENTS CAREFULLY:

Puppy or unvaccinated dog's exposure to germs and other dogs must be limited until they have had at least their first two sets of vaccinations. Vaccines are not effective immediately, so we allow at least a week past the 2nd vaccination date for better protection. Exposure to areas where other dogs have walked and gone to the bathroom will increase the pups risk of contracting disease.

       Until that time,

      • The pup must potty on potty pads or newspaper.
      • The pup must not be placed on the floor inside, or on the ground, grass or dirt outside.

MAKE SURE YOU UNDERSTAND THESE PUPS MUST BE HELD AT ALL TIMES IF NOT IN A DESIGNATED KENNEL OR PLAYPEN WHERE PRECAUTIONS HAVE BEEN TAKEN TO PROTECT THEM FROM DEADLY VIRUSES.

    • I certify that the information provided on this application is complete, accurate, true and correct, to the best of my knowledge. I understand that providing false information may void this application and result in revolving volunteer privileges. I hereby authorize Once Upon a Tail to contact my parent, legal guardian, or other party listed above for further information and/or records.
