WAIVER OF RESPONSIBILITY & MEDIA RELEASE
I understand the normal risks from playing tennis and hereby release the hosting facility, the professionals and any person involved from any responsibility for any damages, losses or injuries sustained in conjunction with this program. I hereby waive, release and forever discharge USTA, LTA, your CTA and the participating facilities, their officials, employees, agents and support personnel from any loss, cost, or expense, claims or demands of any and every kind which I may now have, may acquire while participating in the "Tennis Apprentice" tennis program, or may hereafter have for any and all damages, losses, injuries, cost or expense which may be suffered or sustained by me in connection with participation in the “Tennis Apprentice” tennis program. In addition, I authorize and grant permission to NOMATA, LTA, and USTA to use my photographic image for any video, electronic, or non-electronic form of media in order to promote the “Tennis Apprentice” program.
