Definitions:

"Application Organization" - the entity/business/individual that is listed on the application document.

"Consultant" - Individuals who will assist with the preparation of the application and may be involved in subsequent administration activities or performing work after the grant award. This includes application preparers, researchers, consultants, third-party participants, or sub-recipients.

NOTE:

Completing this form implies that the "consultant" individuals to be added to the document have been selected via the appropriate and applicable procurement process. (this may vary depending on TBD program, so reach out to staff for specific questions).

For CDBG Programs: Submitting this form prior to formally selecting the external grant preparer will result in an apparent conflict of interest, prohibiting the Consultant from participating in the implementation of the grant, if awarded.
