Treatment Rules

 

I am authorizing Get it Girl Beauty LLC, its representatives and service providers to perform the beauty services I have booked. I understand that in order to avoid injury/discomfort, I must lay still during the application with my eyes closed. If there is any discomfort/irritation I will notify Get it Girl Beauty LLC immediately.

Risk Of Allergies

 

I am aware of the procedure and what to expect during the appointment. I understand there are risks for an allergic reaction and/or irritation, and have previously made my service provider aware of any health/sensitivity issues I have experienced.

Limitations

 

I understand that when coming from a previous lash artist or esthetician cannot guarantee that we will be able to work over someone else’s work if there is ANY damage created to the natural lashes or skin. For lash extensions we will recommend removing the eyelash extensions immediately and rescheduling for a brand new full set.

Photo Release

I authorize Get It Girl Beauty LLC, its representatives and service providers to photograph my eyes/face to promote for the business if necessary. I will notify Get It Girl Beauty LLC not to photograph if I am not comfortable with the photos being used for promotional, marketing purposes.

Acknowledgment of Care Instructions

 

I understand the longevity of my eyelash extensions are directly correlated to how they are cared for and treated after leaving my appointment. I will not get my lashes wet for 4-6 hours in order for the adhesive to cure completely and adhere properly to the natural lash. I have been made aware that the following things can prematurely shed the eyelash extensions, and potentially cause damage to the natural lashes: Steam, excessive heat, or saunas; Oils of any kind including make up, skin care, and natural oils produced by the body; perming, curling, tinting eyelash extensions; pulling, rubbing, touching, or sleeping on extensions. In order to keep my extensions symmetry and fullness, I must have them filled every 1-3 weeks.

Cancellation Policy

 

Same day cancellations and No-shows will result in a charge of 100% of the service fee. If you cancel with a 24-hour notice you will be charged 50% of the service fee.

*The only way you will not be charged a cancellation fee is if you cancel 48 hours before your appointment!

I give Get It Girl Beauty LLC permission to keep an active card on file and to charge my card even if I don't show up or do not cancel per the cancelation policy. I understand if I miss an appointment and the charge does not go through, I will not be able to book a future appointment until Get It Girl Beauty LLC has been paid the prior fee owed.

Waiver

 

By signing this release waiver agreement, I, the client below, certify that I knowingly and voluntarily release Get It Girl Beauty, as well as, its representatives and service providers from any and all claims for injury arising from the application and use of semi-permanent eyelash extensions, Hyaluron Pen Filler, or facial beauty services. Including any damages relating to known or unknown complications which may arise during or following the application/beauty service process, including but not limited to claims for negligence. I further release and hold harmless Get It Girl Beauty LLC, its representatives and service providers from any claims relating to preexisting conditions I have not revealed, or changes to those conditions after the procedure. I agree that any disputes between Get It Girl Beauty LLC, its representatives and service providers shall be settled by arbitration administered by the American Arbitration Association in accordance with its commercial arbitration rules, and judgement on the award rendered by the arbitrator(s) may be entered in any court having jurisdiction thereof. The prevailing party shall be entitled to reasonable attorney fees and arbitration costs incurred in connection with enforcing this agreement.

I certify that I have read and fully understand this Customer Waiver & Release Agreement. I hereby authorize Get It Girl Beauty LLC and/or its representatives and service providers to provide beauty services, including esthetician services and apply semi-permanent eyelash extensions to my own natural lashes, in accordance with the terms and conditions set forth in this consent agreement.
