Special Event Application
Application Fee: No fee for submittal; other service fees may apply. A completed Special Events Application, with required documents such as proof of insurance (certificate of liability and additional insured endorsement page) and a plot plan, shall be filed with the Community Development Department a minimum of 30 days before the date of the event. A business license may be required. Inspections and conditions will follow upon approval. Please read city policy.

A plot plan is required for application approval.
Did you submit a copy of your event waiver of liability?
A copy of the event waiver is required for each active participant. The city must be named on your waiver. See procedure/policy section.
Did you include insurance coverage?
Insurance is required for application approval. The City of Loma Linda must be named as additionally insured.
Is the event a fundraiser?
Example: Donations, tickets, drawings, etc.
Will the event have amplified sound?
See Chapter 9.20 Noise Regulations of LLMC.
Is water access needed for the event?
Access is not available in all locations.
Is electrical access needed for the event?
Access is not available in all locations.
Is a sign or banner required for the event?
A sign plan (diagram) or permit may be required.
Will alcohol be sold or consumed at the event? If yes, please attach a copy of the ABC permit. Show the location and hours that alcohol will be served on the plot plan.
Will food be prepared or sold on site? If yes, please attach a copy of the San Bernardino County Health Permit & Food Handler's License. Fire Prevention must inspect the equipment before the event.
Will temporary structures be used? If yes, please show on the plot plan the location and dimensions. Tents over 400sqft. will require a tent permit from Fire Prevention.

Does the event require a street closure or block driveways? If yes, please show on the plot plan the street/driveway closure. If the closure is extensive, please submit a separate traffic closure/detour plan.
Will any modification be done to the site or building?
Example: new walls or new electrical?
Will any products or services be available for purchase at the event?

Was a licensed company hired to develop and manage the event's security plan? If yes, please provide a copy of the security company's private patrol operator's license issued by the State of California.
