7. If enrolling into the Diploma of WHS - a copy of your Certificate IV in WHS
   
   
8. Apply for credits from other qualifications
   If you want to apply for credits from other Certificate IV, Diploma or Advanced Diploma qualifications that you have previously completed, you will need a copy of your transcript for the relevant qualification(s). This needs to show the code and names of the units you completed.

Furthermore, I acknowledge that the following requirements will be adhered to within the commencement and completion dates in order to be issued my qualification and results:

1. Participate in recorded phone conversations to assist in verification of identity for Auditing purposes.
2. Provide electronic copy of Photo ID or formal documentation stating my full name and current address.
3. Will complete the ‘completion student survey’ if employer has paid my student fees, assist Global Training Institute to gain an completed Employer Survey form.
4. I agree to pay my student fees and I understand that I will not be receiving my qualification until my student fees are paid in full.
5. Should I not complete the course prior to the date agreed (set by my funding body or advertised as Course Length/Access on website) and I wish to still complete it after this time to gain my qualification, I agree to pay Global Training Institute a new negotiated amount.
6. Should I wish to withdraw at any stage from the course, I agree to sign a ‘Withdrawal Form’ and complete 1-5 listed above. I understand that I will only then receive a Nationally Recognised Statement of Attainment for the successfully completed units.
