Instructions: Use the forms below to add or correct information for individual members. If you are submitting information for more than one member, use the + sign to add rows as needed; each row should have information for just one member.

Use the Additions form to add current Harmony, Inc. members to your roster. These members must hold a current membership in Harmony, either as an Associate or Affiliate or with another chapter. Indicate the member's previous status (Associate, Affiliate, or the name of the former chapter) in the Transferred From field.

Use the Corrections form to make corrections for members currently listed on your chapter's roster. If the correction is a name change, please include the former name (in parentheses) along with the new name.
