The respite voucher program requires JF&CS staff to communicate with home care companies and adult day care centers in order to initiate services for clients. Only pertinent client information regarding in home respite services with home care companies or out of home respite services with adult day care centers will be exchanged. By printing your name and signing below you authorize JF&CS staff to communicate with in home respite providers (home care companies) and out of home respite providers (adult day care centers) to initiate services.
