Use this to update any legally binding information you've given us before, for example:
- Add or remove names from the list of people we're allowed to share your confidential information with
- Change your psychiatric living will and preferences on what we should do in an emergency situation
- Change the email address you want us to contact you at
Note that none of these changes will go into effect until we've sent you an email at the address on file to confirm. If you've lost access to that email account, please message us and we'll try to sort it out.
You don't need to use this form to update information that isn't legally binding, like your insurance, address, or medication list. You can just email your doctor or let them know the next time they see you.
