CLIENT THERAPIST AGREEMENT The therapy relationship is a collaborative one. You will be working with one of the team's well trained and excellent clinicians. You and they will establish goals together and work to accomplish them.
In order to facilitate your work, there are a number of policies and procedures that need to be followed. They are outlined below:
All clients are seen as part of DAVIDOVICS THERAPY GROUP PLLC and adhere to all policies and procedures as in the private practice of Marcy Davidovics, LCSW.
After you are assigned a clinician, you and they will set the session hours together and they become the contact person from then on.
However, all payments will go through DAVIDOVICS THERAPY GROUP PLLC. All Contact information for Davidovics Therapy Group is above.
OFFICE HOURS We cover most hours of the day and evening. We are highly efficient about
getting back to you in a timely manner and set you up with a therapist that meets your needs.
FINANCIAL AGREEMENT & CANCELLATION POLICY
The fees for private clients vary depending on multiple factors, including: clinician’s specific training, years in the field, client’s ability to pay, etc.
DAVIDOVICS THERAPY GROUP PLLC accepts credit cards (preferred method), checks, cash, and venmo and payment is expected on the day of treatment.
We do NOT accept cash pay platforms such as PayPal, Zelle. etc.
Because your appointment time is held exclusively for you, we have a general 48-hour
cancellation policy.
PHONE CALLS, TEXTS, and EMAILS
Your therapist will set her/his own protocol around phone calls, texts, and email and will
communicate with you accordingly. There may be some forms to sign as well.
Of course, if a situation is life-threatening, please contact 911.
CONDUCT OF THERAPY
We all adhere to the Code of Ethics as a clinical social worker (LCSW and LMSW) and to the laws of New York and nationally as they pertain to client-therapist relationships.
CONFIDENTIALITY
The content of all professional interactions in my practice and therapy group will be held in
confidence unless you waive this confidentiality in writing.
However, confidential information can be subpoenaed by court order, although it doesn’t
happen often.
Additionally, information concerning child or elder abuse, physical violence, or threats to others or self is REQUIRED BY LAW to be reported to the designated authorities. Therefore, this kind of information will not be kept confidential.
CONFIDENTIALITY WAIVER Under federal law, you have the right to confidentiality with regards to treatment and treatment sessions you receive. However, to provide therapy services with a holistic approach, it is common practice to ask clients to allow the therapist to speak with other service providers they are seeing such as: psychiatrists, psychotherapists, medical professionals, etc. Of course, this will only happen with your approval.
For those clients under 18 years of age: parents/guardians may be asked to give consent for me or your therapist to speak to your child or teenager’s school or other specialists.
