    Please fill out the following form with as much detail as possible. If I am able work with you, I will offer you an appointment date. Once an appointment date has been offered, you will have 24 hours to to pay a $100 deposit to secure your appointment. Your deposit will be applied to the cost of your tattoo. If your tattoo will require multiple sessions, the deposit will be applied to the final session. Deposits are non-refundable. However, if you need to reschedule your appointment, your deposit will still be honored as long as you provide 48 hour notice of your need to reschedule. *** Please provide as much description as is reasonable to ensure I get as close to your vision as possible.***You will not see your tattoo design before your appointment.*** Any small changes that may be required will be handled on the spot.
