Payment agreement, please read carefully, before purchase:

1. The online course materials are approved by the city and state and structured to prepare participants to test for the nationally accredited ANSI approved Food Protection Managers examination. Once this exam is passed the participant will obtain the required (City or State) Food Service Managers Certificate.

2. This BUNDLE package includes the 8 hour Online Food Service Managers training, Virtual Online Exam and City Food Service Managers license fees.

3. Advocate Food Safety Solutions will submit the necessary paperwork with the city of Chicago, after successfully passing the exam with a 70% or better.

4. The City Food Service Sanitation Managers Certificate license will take 8 to 10 weeks to be delivered. This applies to most food retail licensing matters and cannot be expedited.

5. The online course can be viewed via tablet, PC and smartphone. The larger the screen, the better application.

6. The entire online course must be completed within 20 to 30 days, from date of course activation, at which time, the course will be deactivated. If the course is deactivated there will be no refunds.

7. If the entire course has been activated/entered within the 20 to 30 day period with more than 45 mins or 30% course contact time, the course will not be refunded.

8. In the event of a refund: 60% of the fee, will be credited to the credit card on file.

9. The final online exam is a nationally accredited exam provider that must be passed with a 70% or better, to receive the Food Service Managers certificate.

10. The online virtual exam will be conducted in a Google/Zoom platform and must be taken on a tablet, chromebook, personal computer, etc. with a built in camera and microphone.

11. The examination will be conducted by using 1 of the nationally accredited and approved ANSI Food Protection Managers Examine providers. The ANSI examination is approved in all 50 States.

12. Payment will be collected via Paypal, however, there is NO NEED to set up a paypal account.

13. Any exam RESCHEDULES/MISSED OR NO SHOWED EXAM SESSIONS, will incur a reschedule fee.

14. All exams must be rescheduled within 30 days of the original exam session.
