PARTICIPATION CERTIFICATION. I declare that I am in need of Educational Opportunity Center (EOC) services and all information provided is accurate. EOC has my permission to verify this information. I understand that the submission of false information can result in my termination from the program. I authorize the release of test scores, transcripts, admission, financial aid, enrollment and other relevant information that could assist EOC in meeting my academic need. I have been advised of the above statement and I consent to participate in the SLU EOC program under the conditions explained.*
