    • We may need to talk to the agency that referred you, previous employers, referees or education organisations to gather information about your knowledge, skills and experience in relation to new work opportunities. We will always ask for your permission first.

    • We may also need to talk to employers about possible jobs but will ask for your permission first.

    • Ministry of Social Development (MSD) fund this service. We are required to provide them with information about the service we provide to you and to advise them if you are employed through this service. MSD may contact you to confirm you have received the service from us.

    • Your personal information is stored by Habit Health on a client database. We have security procedures in place that protect against loss or misuse of your personal information.

    • You can access the information we hold about you at any time, you just need to ask.

    • You can ask us at any time for us to change any information we hold about you that is not correct.

    Note: You will not be eligible for the service if you:

    • Are still enrolled at school

    • Qualify for vocational rehabilitation through the Accident Compensation Corporation (ACC)

    • Are being provided vocational services through the Ministry of Health
