You are registering for Radi8 2021!

An activities based group for children in years 4-6 that runs on selected Friday evenings from 6:30-8pm.

THIS PERMISSION FORM IS FOR ALL RADI8 EVENTS IN 2021.

Drop off and pick up from 176 Wattle St Malvern. Some events may involve a walk off site. Specific details of each event will be emailed to you in the week prior.

Which events will you be attending?

To simplify the registration process we'll assume you are attending all the events. However, we understand that you may not be available for a few of them and that is ok.
Informed Consent and Acknowledgement

By submitting this form I give my approval for my child’s participation in activities prepared by Radi8 (Trinity Church Unley) while under their care at each of the different events checked above.

I understand that in the case of an accident or emergency my child will recieve first aid from a designated first aid officer and an ambulance may be called where deemed necessary.

I understand that I my child must be signed in and out of the Radi8 program by my myself or an adult I deligate this to.
Privacy

All the information recorded on this form is collected and managed in accordance with the Anglican Church Privacy Policy. This information has been collected for the primary purpose of running and managing Radi8 events through our church office and will not be given out, sold, or used for any other purpose.

Photography: At this stage we have no intention of taking photos of any children. If this changes we will seek written concent from parents/caregivers.
