This order form is used for obtaining database information, whether for information purposes only or in order to obtain a Final Count of mailable addresses to process a mailing job. NOTE: A Final Count must be received from the Database Specialist, on EVERY mailing - even if using an Agent's Personal Database, before the job can be processed through COR's Print & Mail Center.
