I consent to the procedure of EyeLash Perming. I understand that this is a treatment, not a science. I agree that if I am not satisfied for any reason will inform the staff at Petite Spa in a timely manner. I understand that this procedure is purely cosmetic, and I am making this decision to have this service performed under no pressure, and not under the influence of alcohol, or any mind altering medications.

Process:

Eyelashes are temporarily glued to an eyelash perm rod, a diluted perm gel solution is applied. Which is followed by a cream neutralizer to deactivate the perming process. We then use a lubricant to remove the glue, and follow with a mild detergent to cleanse the area. This procedure should result in curled lashes for up to 3 months. As the eyelash hair are replaced with new hairs, the process should be repeated.

 *It is recommended that you do not come in with mascara on, or have used an oil based makeup remover within the last 24 hours. If you have done either it is important to inform your esthetician.

 Risks: Damage to the eye, and surrounding skin can occur. Stinging, tingling, burning sensation, and blindness are potential risks to this procedure. I will inform my esthetician should I fell anything uncomfortable at any time through out this procedure.
