If you would like to register more than one participant, you must complete another registration form for each participant you wish to register. FIELDS THAT ARE NOT MARKED WITH AN ASTERISK (*) ARE OPTIONAL AND NOT REQUIRED. Photo Release Waiver*

I, the Participant or Parent/Guardian hereby give Team First Elite AAU Basketball and its representatives, the absolute and irrevocable right and permission to use, reproduce, copyright and/or publish the likeness in photographic portraits, pictures, and/or videos of me/my child for the tryouts. I acknowledge these publications include but are not limited to, recruitment publications, newsletters, newspapers, magazines, videos, websites, art, advertising, trade, or other lawful purpose whatsoever at any time, without payment or other consideration. I acknowledge that all such images and prints shall be the property of the Team First Elite Organization. I hereby waive any right that I may have to inspect and/or approve the finished product or the advertising copy or other printed material that may be used in connection therewith, or he use in which it may be applied. I hereby release, discharge, and agree to hold harmless the Team First Elite Organization from any and all claims, actions and liability relating to the organizations use of such photographs. I have read and understand the contents of the release and confirm that I am the Participant or Parent/Guardian and that I am signing this release intending to be legally bound by these terms.

REFUND POLICY

NO REFUNDS WILL BE GIVEN AFTER THE START OF THE TRYOUTS REGARDLESS IF YOU ARE SELECTED FOR A TEAM FIRST ELITE BASKETBALL TEAM OR NOT. To receive a full or partial refund, all requests must be submitted to teamfirstelite2022@gmail.com. Request must be made at least 24 hours prior to the scheduled Tryout Date. Refund requests within 24 hours of the scheduled Tryout Date are not subject to be refunded and payment will be invoiced if not previously paid. Participants that sustain injuries while participating in a Team First Elite event are not subject to refund. Injuries that are sustained outside of Team First Elite Basketball or with another team prior to the Tryout Date are not subject to a refund. TEAM FIRST ELITE IS NOT SUBJECT TO ISSUE A REFUND IF A PARTICIPANT WILLINGLY OPTS OUT OF TRYOUTS. I hereby release, discharge, and agree to hold harmless the Team First Elite Organization from any and all claims, actions and liability relating to the organization's REFUND POLICY. I have read and understand the contents of the REFUND POLICY and confirm that I am the Participant or Parent/Guardian and that I agree to the REFUND POLICY and that I am signing this release intending to be legally bound by these terms.
PAYMENT TERMS

You have 48 hours to make full payment after successful submission of your registration. If not, your registration is void.

Payments are 100% refundable if you cancel your registraion within 48 hours after making your registraion payment. Otherwise your registration payment is NONREFUNDABLE.

To cancel registration and/or request 100% refund, Email: info@teamfirstelite.org. Successful refunds automatically forfeits your (or your team) registration.
