Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Via Papera LLC, operator of Arte al Sole programs, during the selected camp. In exchange for the acceptance of said child’s candidacy by  Via Papera LLC, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Via Papera LLC. and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Via Papera LLC, including all coaches and affiliates, teachers, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities. 

I hereby voluntarily, at my own risk, sign this Informed Consent and Acknowledgment and acknowledge and understand that there may be risks associated with the activities, which have been fully explained to me. I hereby agree to abide by all rules, instructions, policies and procedures imposed by the Releasee. I further agree to indemnify and hold harmless the Releasee, its employees, agents, officers, from and against any and all liability incurred as a result of or in any manner related to my participation in the activities.

I hereby certify that I am of legal age and competent to execute this Informed Consent and Acknowledgment, that in doing so of my own free will and accord, voluntarily and without duress, and that I do so intending to bind myself, my executor, my heirs, and administrators or assigns to the fullest extent.

I have read and understood the foregoing, and acknowledge my consent to the terms of this Informed Consent and Acknowledgment by signing this Consent.

Medical Release and Authorization

As Parent and/or Guardian of the named student, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named athlete. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to the Via Papera LLC  and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Media Release

I hereby authorize Via Papera LLC to publish the photographs and videos taken of me and/or the undersigned minor children for use in the Arte al Sole printed publications, website and training purposes.

I release Via Papera LLC from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorize Arte al Sole to use their photographs, videos and names.

I acknowledge that since participation in publications and websites produced by Arte al Sole is voluntary, neither the minor children nor I will receive financial compensation.

I further agree that participation in any publication and website produced by Arte al Sole, its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.

Confirmation

BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.

Cancellation Policy: Arte al Sole activities bookings are cancellable for a full refund up to 30 days prior to the activity date. Bookings are non-refundable after 30 days. Custom or private programs have individual cancellation policies according to the program.
