EVENT WAIVER & LIABILITY:

*Waiver, indemnification and release of liability agreement authorization and permission to use name and likeness for the publicity and display of images on the Got Five(G5),LLC Tournaments,Events & League website and social media. This liability form is also excluding Got Five(G5),LLC Tournaments,Events & League, Rancho Elite Basketball Program,Ontario High School,Patriots High School,Rubidoux High School,Chaffey College,Trifytt Sports,Get It Done Arena,Kevin Alexander, Saabrina Rivera and location of event and or games and tournament from any liability of injury and accident of player or persons affiliated with the said listed program.

By signing this waiver you are in agreement with all that is said within this information document. ***ONLY 2 COACHES PER TEAM NO EXCEPTIONS,COACHING CREDENTIALS MAY BE REQUESTED UPON VERIFICATION OF COACHING CHECK IN,ALL OTHERS WILL BE ASKED TO PAY AT DOOR***

Sportsmanship Agreement: I (Director/Head Coach) understand that I am solely responsible for the sportsmanship of me myself, assistant coaches, athletes, staff and stats personnel, athlete’s parents, and / or any fans or person otherwise associated with my school / team.

This includes, and is not limited to, sportsmanship towards other coaches, other athletes, the officials, and / or any person participating / involved with the Got Five(G5),LLC Tournaments,Events & League Staff Memebers & Directors.

We,as a team, agree to display proper sportsmanship throughout the duration of the league, and understand that failure to adhere to such standards could result in removal from the Got Five(G5),LLC Tournaments,Events & League.

NO refunds will be issued upon removal, and your team will still be responsible for ensuring all payment fees are paid prior to the due date stated above.

NON Payments will STOP,STALL AND FORFIET GAMES!

Signature: By signing below, you are taking full responsibility for the Waivers to participate in the said event as well as for any and all financials due prior to the start of your games. If your payment has not been made prior to the event your do understand that your games may be forfeited and or cancelled. Any and all prior arrangements must be advised to hosting director. There is a 48 hour "Drop Out" requirement. If you do not advise a team not participating in the event prior your fee and or deposit for teams listed above will not be credited or refunded and this may affect any further participation in the Got Five(G5),LLC Tournaments,Events & League Further consideration will have to be considered by hosting director.
