    A home repair program for low-income homeowners.
  •  Please read the enclosed information carefully and fully, and remember:

    • You must submit all requested documentation listed below.
    • You must completely fill out the repair application. All applicants must sign and date it.
    • Your repair request must be necessary for the home to remain safe, healthy, or improve accessibility.
    • All applicants must be listed on the Deed AND must be living in the home (or suitable documentation provided demonstrating why an applicant is not living in the home).
    • Turning in an application does not equal funding approval.
    • Do NOT let repair work begin until you have a letter of approval from our agency.

    Other than online, applications may be submitted in person delivery or by mail or email:

    Have the following documents ready to upload:

    1. Applicants:  Proof of Identity (Driver’s License or other current ID)
    2. Applicants:  Copy of the Warranty Deed for house needing repairs
    3. Everyone:  Proof of Income for every adult member living in the household (SS letters, most recent 2 pay stubs, child support, retirement, etc.)
    4. Applicants:  Most recent 2 months bank statements on all accounts, all pages
    5. Applicants:  Current homeowner’s insurance policy
    6. Applicants:  Most recent mortgage statement, if applicable
    7. If a veteran resides in the house or you are the widow of a veteran, please turn in a copy of the DD-214 form

  • If your home is a manufactured home, is it on a permanent foundation? (Permanent foundation means concrete blocks constructed of single, open, or closed-cell concrete blocks, with a nominal dimension of at least 8 inches x 8 inches x 16 inches, the concrete blocks must be stacked with their hollow cells aligned vertically, mortar is not required. Capped-hollow block piers must be caps of solid concrete or masonry at least 4 inches in nominal thickness, or hardboard lumber at least 2 inches nominal in thickness; or be corrosion-protected minimum one-half inch thick steel; all caps must be of the same length and width as the piers on which they rest. Any gaps that occur during installation between the bottom of the main chassis beam and foundation support system must be filled by: Nominal 4 inch x 6 inch x 1 inch shims to level the home and fill any gaps between the base of the main chassis beam and the top of the pier cap; shims must be used in pairs, as shown in Figures A and B to § 3285.306, and must be driven in tightly so that they do not occupy more than one inch of vertical height. Tie-down straps must be 1 ¼ inch x 0.035 inch or larger steel strapping. Underpinning / skirting must be around entire base of home unit.)*
