Eligibility Information for home repair:

• The home must be located in Jefferson, Grainger, Hamblen, or Cocke County.
• The homeowner must occupy the home and has lived in the home at least three (3) years.
• The person applying for home repair must own the home, have a life estate, or is purchasing the home with a traditional mortgage.
• The homeowner can't have filed a bankruptcy within the last five (5) years

Required Documentation:

• Copy of current mortgage statement (if there is still a mortgage)
• Copy of the title if a mobile home
  • If the mobile home is in a mobile home park, a copy of the most current lot rent receipt
• Copy of the deed showing the home repair applicant as property owner
  • copy of death certificate if two people are listed on the deed and one is deceased
  • A Life Estate showing the resident of the home can make decisions on the property
  • a Quit Claim showing the property has been signed over to the resident
• Copy of current utility bill
• Proof of Income for every person in the home
  • S.S./SSDI/SSI amount award letter received at the beginning of the year showing the monthly amount to be received for the current year
  • Child support documents
  • Retirement/survivor benefits/VA
  • Employee payroll statements for no less than one month or previous year's IRS tax return if self employed
• Copy of Paid Property Tax receipt

  • IF THERE IS A MORTGAGE, A COPY OF THE DEED CAN BE OBTAINED AT THE COUNTY COURTHOUSE
• CURRENT UTILITY BILL
• PROOF OF INCOME OF EVERY PERSON IN THE HOUSEHOLD
• STAMPED PAID RECEIPT OF CURRENT YEAR PROPERTY TAXES

I grant Applachian Outreach Ministries, its representatives, and employees, the right to take photographs of me, my family and my property.  I aughorize Appalachian Outreach Ministries, its assignees and transferees, to copyright, use, and publish the same; in print and/or electronically.  I agree that Appalachian Outreach Ministries may use such photographs of me and/or my family with or without my name and for any lawful prupose, including for example such purpose as publicity, illustration, advertising, and web content.
