Volunteer Disclosure and Consent to obtain Criminal History and Driving Record

for coaches and any other volunteers

I, hereby authorize the City of Mauldin to obtain background check information regarding my criminal history and vehicle driving record as

necessary in making decisions regarding my application. By granting this permission, I hereby release any agency or person from any civil or criminal liability, which would ordinarily attach to the confidentiality of this record information.

I understand that as a volunteer with the City of Mauldin Parks and Recreation Department, I will be working directly with minors and the City of Mauldin has a legitimate interest in only having volunteers who satisfactorily pass a background check. A list of disqualifying guidelines is available. Any disqualifications will be reported to the applicant.

The background Check may include, but not limited to: Criminal History Sex Offender Registry Driving Record

The City of Mauldin Recreation Department has adopted the following criteria for exclusion of volunteer application:

• Sex Offenses - regardless of the amount of time since the offense
• Felonies - any felony violence regardless of the amount of time since offense
• All felony offenses other than violence or sexual offenses within past 10 years. Examples include drug offenses, theft, embezzlement, fraud, child endangerment, etc.
• Two or more misdemeanors within past 5 years. Examples include driving under the influence, simple drug possession, drunk and disorderly, public intoxication, possession of drug paraphernalia, fraudulent checks, or any other misdemeanor that would be considered a potential danger to children or directly related to the function of that volunteer.
