If you child has any allergies or a chronic medical condition (such as food allergies, asthma or diabetes) you must fill out the following forms and return them with any related medications BEFORE your child's first day at Woodside Montessori Academy. Your child will not be able to start at Woodside until all appropriate paperwork is in and medications are onsite.

Food Allergy Action Plan*
Individual Health Care Plan*
Anaphylaxis Emergency Acation Plan

Some doctors have these forms on file and can fax them to the office. The office fax number is 508-590-6644.

*These forms must be signed by a parent or guardian AND the doctor.
