    Cat Adoption Application
  • Applicants must be 18 years of age or older to adopt from Fresh Start Animal Rescue League.

  • Before starting this application; are all pets currently in the home spayed/neutered and current on their vaccines? If the answer is no, please do this before applying for one of our pets. We require all animals currently in the home be spayed/neutered and current on their vaccines prior to adopting from us. Please complete all the information on the following pages. Missing information may delay the adoption screening process. Completion of this application form does not guarantee that FSARL will place one of our pets in your care. Completion of this questionnaire authorizes FSARL Members to verify any information contained herein, including verification of medical records of any present or prior pets treated by the veterinarians listed. Completion of this adoption process is an indication that you have read, understand and agree to abide by our Animal Adoption Agreement and that the information contained within this application is accurate and not misleading in any way.  All adoptions are at the sole discretion of FSARL and we have the right to refuse an adoption if we see fit.

  • Please give careful consideration to adopting a cat. Cats are not toys or short-term commitments. Make sure your lifestyle allows the time, patience, training, and expense this cat will need over the years. Also keep in mind that rabies vaccines are required BY LAW, even for cats. All cats require a vet visit scheduled and verified prior to taking them home. Yearly wellness visits are a must and will be followed up upon.

  • All pets will be spayed/neutered, vaccinated, FIV/FeLV tested, treated for parasites and microchipped prior to adoption if age appropriate. If adopting a kitten under 6 months of age they will be adopted out under a sterlization contract that will be followed up upon. A breach of any of our contracts is grounds for removal of our animals. By completing this form you understand and agree to all requirements.

  • Please provide us with at least 2 personal references NOT related to you. Also, please use references that do not live at the same residence. Your personal references should be able to have an idea of what kind of pet owner you are. You can use: coworkers, neighbors, friends, people who would know about your animals. 

     

    Again, your references cannot be related to you and should reside in separate homes from you and eachother.

  • If you rent, can you provide us with your landlord's contact information below? Also, could you let them know we will be giving them a call?

  • We will be speaking to all household residents to confirm everyone is aware of this application process and wants to have a cat added to the family. Please do not submit this application if you are "trying to talk someone into" adopting this cat or think "once they meet the cat, they will want one" or "I'm getting this cat as a birthday present for them." In those scenarios, you are welcome to email us to discuss the cat, but we would like all adults in the home to be involved in that email discussion.

  • PLEASE READ: The adoption fee covers fully vetting the dog, which includes: microchip, spay (for females), neuter (for males), FIV/FeLV testing, fecal test (to check for worms and intestinal parasites), rabies vaccination, distemper combo vaccination, dewormers, a dose of flea/tick preventative (which will kill fleas/ticks and provide a month's worth of protection against them) and grooming if needed. Your adoption fee will be the cost of all of this vetting. You are required to take your cat to the vet within two weeks of adoption for a fecal retest to make sure all intestinal parasites are gone. We do not declaw our cats nor do we condone it. If you are having issues with your cat clawing or scratching you should contact us or your vet immediately. By signing this contract you agree to never declaw your cat unless it is recommended by your vet and we have approved it. Adoption fees are considered donations and if you feel you need to return this cat for any reason, you understand donations are NON-refundable. By signing this document I agree to all stated above and I understand everything I have read.
