Thanks for ordering from Blue Moon! Please complete this order form below by Sunday at 2 PM for delivery in the upcoming week. If we receive the order after Sunday, your delivery may be delayed until the following week. 

We will confirm with you over email and bring an invoice in person when we deliver, as well as send a digital invoice over to you by email with the ability to pay online or by check. 

For orders under $500, there is a $40 delivery fee; for orders over $500 - $999, there will be a $25 delivery fee, and we are now offering FREE DELIVERY on orders over $1,000. This applies to most delivery zones - we will let you know if your region falls outside of these standard rates.
