Mentor Agreement

Please read the following statement and sign this agreement below before submitting.

I acknowledge that by applying to be a mentor, I am committing to:

Attend a Mentor training.
1. Be matched with a selected mentee for a period of 3 months (minimum) from the commencement of the contract.
2. To meet with my mentee at least twice a month.
3. Help identify my mentee's goals and provide thoughtful advice and feedback to help your mentee make progress toward those goals.
4. Understand that confidentiality is crucial in a mentoring relationship and agree to work with my mentee to establish our common expectation of confidentiality.
