The undersigned hereby represents that all of the above information is true and makes said representations for the purpose of obtaining credit from K-B Commercial Products.
Agreement and Terms*

The undersigned agrees to pay all bills for merchandise received from K-B Commercial Products according to the terms of the sale and for failure to do so, agrees to pay all costs of the collection, including a reasonable attorney's fee, with or without suit, together with a service charge on any unpaid balance at the rate of 1.75% per month (21% per annum) unless otherwise agreed upon in writing.

Personal Agreement

In consideration of K-B Commercial Products extending credit to the above, the undersigned does hereby personally guarantee to pay K-B Commercial Products payment for merchandise received from K-B Commercial Products. The liability of the undersigned shall not be affected by any indulgence, compromise, statement, extension, or variation of terms and notice presentment, dishonor is hereby waived. Extension of time for payment is solely at the option and discretion of K-B Commercial Products.
