Photography Disclaimer: By signing up, you agree to the DOUGLASS VISIONS COMMITTEE, INC term of use of your likeness. You agree to, and represent and warrant the following: I give Douglass Visions Committee permission to photograph my likeness, and 1) all adults appearing in a photograph gave unrestricted written consent to photograph them and license use of the photographs to Douglass Visions Committee, and 2) the parent or guardian of each child under age 18 gives Douglass Visions Committee unrestricted written consent to photograph the child and license use of the photograph to Douglass Visions Committee.

Further, I, for myself and my heir, executors, administrators, and assigns, hereby release, waive, and discharge Douglass Visions Committee, Inc., and its officers, directors, employees, agents, and volunteers of and from any and all claims which I or my heirs, administrators, and assigns ever may have against any of the above for, on account of, by reason of or arising in connection with such volunteer activities, or my participation therein, and hereby waive such claims, demands, and causes of action.
