This checklist is a guideline for apprising all onsite workers of certain minimum safety requirements of this jobsite. All workers should expect to come to a safe jobsite and will be held accountable for following the requirements discussed during this orientation and listed in Kraus-Anderson’s Safety and Health Program and within your own company’s safety program. The goal of this orientation is to foster a collaborative work environment where workers look out for each other to reduce the frequency and severity of worker injury.
1. Review of Safety and Health Program Requirements:
- Safety Data Sheets are required from all subcontractors and are available to all workers.
- All containers need to be properly labeled.
- Emergency management plan review and care sites reviewed.
- All applicable certifications (operator, respirator, rigging, etc must be current and available prior to starting work on site.
- Weekly safety toolbox talks are required with documentation submitted to the project superintendent.
- All subcontractors are required to have their own first aid supplies readily available for use. As a back-up, first aid kits are available in the Kraus-Anderson ("KA") field office.
- All incidents involving personal injury, property damage, near misses and/or failure of safety equipment must be immediately reported to KA supervision so appropriate care can be provided.
- Disciplinary procedures: 3-strike policy or immediate termination for flagrant violations as determined by the project superintendent/foreman.
2. Personal Protective Equipment (PPE) Requirements:
- Hardhats, safety glasses (with side shields), pants and work boots are required at all times without exception. Hi-visibility safety garments (ANSI Rated Class 2), are required per OSHA requirements or at the discretion of the project superintendent.
- Face shields, hearing protection, respiratory protection, protective gloves and other appropriate work clothing are required when applicable to meet all OSHA, KA, owner and subcontractor requirements.
3. Housekeeping, Tools and Equipment:
- Housekeeping will be required by all employees as the day progresses.
- Break areas need to be clean and sanitary at all times. Trash cans with lids are required.
- Proper guards on all tools must be intact and in adequate condition at all times.
- All damaged tools must be tagged “OUT OF SERVICE” and removed from site immediately.
- All electrical, rigging and fall protection must be inspected daily to ensure they are safe for use.
- All cylindrical materials must be racked or stacked and chalked away from primary walkways.
- Exposed nails must be removed or bent over to avoid accidental contact.
4. Trenching and Excavation:
- Inspections of excavations conducted daily and after rain.
- Utility locates must be called 48 hours before you dig.
- Soils must be sloped per soil classification or per specific site requirements.
- Bracing, shoring and underpinning must meet be supervised by a competent person.
- All trenches and excavations 4 feet or greater in depth must have a safe means of egress within 25ft.
5. Operation of Earth Moving Equipment:
- When approaching earth moving equipment maintain proper distance of fifty feet minimum.
- Utilize a visual, voice or hand signal to the operator to approach equipment and the operator.
- Operator shall put equipment transmission in neutral, set parking brake and let approaching worker know it’s safe to approach.
- Do not approach operator from blind spots on equipment.
- Operator shall make routine inspection of equipment and work area including overhead and underground utilities and utilize safe operating procedures.
- All workers on the ground near mobile earth moving equipment shall wear a Class 2 or greater high-visibility garment.
- All earth moving equipment shall be provided with a back up alarm or signal person.
6. Electrical Safety:
- All electrical cords and equipment shall be inspected daily and must be immediately removed from service if defective.
- GFCI’s are required at all times.
- Only authorized electricians are allowed to perform work on primary electrical systems.
- All circuits must be de-energized with appropriate lockout/tagout procedures in place when required.
- Identify power lines and determine proper safety precautions. De-energize / relocate / distance.
7. Fall Protection:
- 100% tie off is required when exposed to a fall potential 6’ or greater and in accordance with all OSHA requirements for applicable scopes of work including but not limited to, the following: steel erection, overhand bricklaying, certain roofing activities and working from ladders or scaffolding.
- Guardrails must be constructed/and maintained per OSHA requirements.
- All openings 2 inches or greater must be covered, secured and capable of withstanding 2 times the maximum intended load and marked appropriately with the word “Hole” or “Cover”.
- All fall protection must be inspected daily and utilized in accordance with all OSHA requirements.
- Impalement protection shall be utilized on all impalement hazards.
- Ladders must be inspected daily, erected properly, and utilized meeting all OSHA requirements.
- Scaffolds must be erected by a competent person and meet all applicable OSHA requirements.
8. Fire Protection:
- All individuals must be trained in the proper use and inspection of firefighting equipment.
- Flammable liquids must be stored in approved containers outside of the building.
- Hot work permits must be strictly followed with a designated fire watch in place.
- Fire extinguishers must be immediately available for all hot work.
- All primary walkways, stairways and means of egress must be free of obstructions.
9. Crane and Hoisting Safety:
- All crane operators, signalers and riggers must submit current training certifications to KA.
- Detailed pick plans must be submitted to KA prior to each applicable scope of work.
10. Acknowledgement:
I acknowledge I have participated in the KA jobsite worker orientation. I was instructed in the recognition and avoidance of unsafe conditions and the regulations applicable to my work environment. I reviewed the jobsite orientation checklist and video. I understand that violating policies set forth in this orientation and/or failing to follow safe work practices may result in immediate discharge. I also understand that this orientation checklist, and all education and orientation I have received from KA, are not intended to contain all health and safety rules and regulations established by OSHA, and/or other applicable law, or the policies of KA or my employer if I am not an employee of KA, and that I am responsible for knowing and complying with all health and safety rules, regulations, and policies applicable to my work. This checklist supplements, and does not reduce or replace, any requirements of the subcontract and/or other contract(s) applicable to my work, and/or the safety policies of my employer.
