If significant changes must be made to this final form I may need to set up an appointment with iHeart Mini Aussies to discuss. Appointments can be scheduled through the Calender Link. Making changes to the form may result in fee-for-service minutes, as described in The Estimated Superbill.
I am aware there is a $100 fee to meet at DAB/MCO or SFB. Fee for meeting at another airport will be determined in writing by iHeart Mini Aussies
I am aware there is an additional fee for meeting inside the airport/terminal due to time spent/parking/fees. This will be assessed day of meeting and will be billed after pick up. There is no guarantee iHeart Mini Aussies can meet inside the airport or go through TSA.
I understand that all flights or travel arrangements should be booked as REFUNDABALE. There are many things that can occur outside of anyone's control. Under no circumstances will iHeart Mini Aussies reimburse for flights/parking/car/hotel/or any travel expenses and under any circumstances.
I am aware that final balance PLUS airport fees or any additional fees are due ONE WEEK prior to travel. Final payments should be done via zelle.
