Lunchtime student contract

You may only use the gym Monday through to Friday during your lunch break. Please answer all the questions below and get a parent, and P.E teacher signatures.

What are your fitness aims and goals *
You will be required to sign in each time you use the gym. You must have correct work out clothes and shoes. Incorrect shoes means no workout. Bring a drink bottle (we do not provide cups). Bring a towel and use it. Make sure you spray down equipment after use and put all equipment away. You will be mindful of other gym members presences by keeping noise down and behaving appropriately. You will not use the gym as a hangout, if you are not exercising you will be asked to leave. Please type your name below if you understand the above. *

AGREEMENT

I, the undersigned, agree that the use of any of the Te Kauwhata community fitness centre facilities is at my own risk. A doctor has assessed any medical condition specified above and clearance has been given. I undertake to comply with all the terms and conditions set out by Te Kauwhata fitness centre. I will follow the centre's health and safety procedures.
