- Photo ID for the proposed supervisor (driver's licence or passport)
- Proof of address for the proposed supervisor (driver's licence or other documentation)
- A letter (on church letterhead) written by your pastor, confirming that the proposed supervisor is suitable (if you are using your pastor as your supervisor, then this letter can simply state that they are supervising your exam)
