Auto Pledge Payment Authorization
Auto-withdrawal via Electronic Funds Transfer (EFT) on the 15th each month. I hereby request and authorize the First Unitarian Society of Milwaukee to withdraw my monthly First Church pledge obligation from the checking or savings account indicated below, via the Electronic Funds Transfer (EFT), provided there are sufficient collected funds in said account to pay the same upon presentation. This authority is to remain in effect until revoked by me in writing, and, until First Church receives such notice, I agree that First Church shall be fully protected in honoring any EFT withdrawal. I further agree that if any such EFT withdrawal is dishonored, whether with or without cause and whether intentionally or inadvertently, First Church shall be under no liability whatsoever, even though such dishonor results in the forfeiture of services.
